Tour the Facility
Call our Admissions Manager to schedule a free tour of our facility.
Schedule your Trial Day
We invite prospective participants to schedule a complimentary trial day with us. This visit allows both you and our team to ensure our program is the right fit. Throughout the day, our staff will regularly check in to ensure the participant feels comfortable, supported, and engaged.
Enrollment Intake
Once the trial day is a success for both you and our team, we’ll guide you through the intake paperwork and requirements. Our goal is to make the process smooth and stress-free, allowing us to handle the details of admission for you.
Fee Schedule
Service Rates & Options:
- Half day (4 hours or less): $75 per day
- Full day (4 to 6 hours): $98
- Extended day (over 6 hours): $15 per additional hour
- Van Service (within 5 mile radius): $10 one way/$20 round trip
- Baths: $25 per use

There is no fee/costs in order to schedule and attend a trial day here at MODS.
The enrollment process for our private-paying participants takes between one to three weeks. This includes touring our facility, scheduling and attending a trial day, and filling out enrollment paperwork.
Yes! Here at MODS, we will gladly work with you, the prospective participant, and their case manager to make the process as smooth and as efficient as possible for enrollment.
