Tour the Facility
Call our Admissions Director to schedule a free tour of our facility.
Schedule your Trial Day
We invite prospective participants to schedule a complimentary trial day with us. This visit allows both you and our team to ensure our program is the right fit. Throughout the day, our staff will regularly check in to ensure the participant feels comfortable, supported, and engaged.
Enrollment Intake
Once the trial day is a success for both you and our team, we’ll guide you through the intake paperwork and requirements. Our goal is to make the process smooth and stress-free, allowing us to handle the details of admission for you.
Fee Schedule
Service Fee Rates & Options:
- Half day (4 hours or less): $87 per day
- Full day (4 to 6 hours): $115
- Extended day (over 6 hours): $18 per additional hour
- Van Service (within 5 mile radius): $15 one way/$30 round trip
- Baths: $35 per use
- Included in tuition: daily exercise, snacks, lunch/dessert, field trips, live entertainment, all recreational activities
FAQs
There is no fee/costs in order to schedule and attend a trial day here at MODS.
Families typically pay out-of-pocket for the adult day care program, and many will coordinate with our finance department for coverage from long term care insurance companies.
We are also happy to enroll adults receiving services via Medical Assistance (e.g. Elder, CADI, DD, BI, etc.) and Veteran’s Assistance.
The enrollment process for our private-paying participants takes between one to three weeks. The same is generally true for participants who use VA funding. This includes touring our facility, scheduling and attending a trial day, and filling out enrollment paperwork.
For participants on Medical Assistance, enrollment takes between three to six weeks, dependent on receiving completed paperwork from Hennepin/Dakota/Ramsey county and the case manager.
